UPMC Home Health is hiring a Clinical Manager for the Telehealth Team. \n


The Clinical Manager position is an on-site position that is accountable and responsible for the overall quality of service, overseeing the planning, developing, organizing, integrating, of clinical care and coordination of assigned patients and designated teams, in support of the organizational goals and commitments.

The clinical manager is a vital position in the organization. This position will support the growth of the use of telemedicine withing the home care setting. This position guides and support staff giving them a solid foundation to provide quality care to our patients. This department is made up of a diverse interdisciplinary group that support patient and family in a wholesome approach. We support our patients and families in their home providing them with education to become come confident with self-care. This is a hybrid role working out of the Cranberry and/or McKeesport office. There will be on-call and weekend rotations as needed.

Responsibilities: \n
    \n
  • Accountable for effective performance and follow-through of all assigned responsibilities and for completing responsibilities within designated (or agreed upon) timeframes. Completes responsibilities in a manner consistent with organizational policy, goals, and values.

  • Oversight of interdisciplinary communication, Referral management, Care Collaboration,
  • \n
  • Facilitates patient care coordination and operations including organization and management within established policies, procedures, standards of care, budgetary parameters, and strategic objectives.
  • \n
  • Provides personnel management functions of all assigned staff, including evaluation of performance and implementation of disciplinary actions as appropriate.
  • \n
  • Supervises and monitors productivity expectations for assigned staff, ensuring coverage of services.
  • \n
  • Participates with the hiring/orientation of qualified employee to meet patient care needs.
  • \n
  • Participates in the after hours on-call process to ensure client care policies and procedures are followed and staffing issues resolved.
  • \n
  • Leads regularly scheduled Clinical meetings and Case Conferences
  • \n
  • Communicates effectively with assigned staff, to monitor the changing needs of the patient and family.
  • \n

  • Current Registered Nurse (RN) license in PA. BSN Required.
  • \n
  • OR
  • \n
  • Licensed Physical Therapist (PT) in state of PA and/or (LPT) where applicable, required. Member of the American Physical Therapy Association preferred. 
  • \n
  • OR
  • \n
  • Licensed Occupational Therapy License in PA required. Certification by National Board for Certification in Occupational Therapy (NBCOT) preferred. Member of American Occupational Therapy Association (AOTA) preferred.
  • \n
  • Management experience preferred.
  • \n
  • One (1) year recent home healthcare experience required; three years preferred.
  • \n
  • Knowledge of Federal, State, and JC regulations
  • \n
  • Knowledge of continuous quality improvement, and leadership process knowledge and skills
  • \n
  • Effective customer relationship skills, interest, and willingness to create and maintain relationships with key customers, particularly physicians and payers.

    Licensure, Certifications, and Clearances:
    NOTE: If BSN not at time of hire, must be enrolled in a program within one year of hire/transfer date and completed within three years of enrollment.

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
