Your Role:

As a Medication History Pharmacy Technician Specialist, you'll play a crucial role in the admission process by collecting medication histories for new hospital patients.

Key Responsibilities:

• Work under the supervision of the clinical pharmacist to ensure accurate medication information including drug name, dosage, route, and administration details.
• Utilize various resources for medication history collection, including patient interviews, existing medication records, and physician documentation.
• Update patient records with allergy, adverse drug reaction, pharmacy preferences, and primary care physician details.
• Document medication histories in the electronic health record promptly and accurately.
• Support the medication reconciliation pharmacist in preparing materials for discharge counseling and medication reconciliation.

Qualifications:

• Minimum of 2 years of retail Pharmacy Tech experience is required.
• Preferably, 1 year of experience in a hospital pharmacy setting.
• Pharmacy Technician certification is mandatory.
