Job Description

This Case Manager role involves coordinating patient care within an acute care facility. The Registered Nurse will float between various units, potentially including the Emergency Department, Intensive Care Unit, Intermediate Care Units, Pediatrics, specialty units, and other areas within the Colby and Pacific campuses. A minimum of one year of experience is required.

Responsibilities:

- Coordinating patient care within an acute care setting.
- Collaborating with the interdisciplinary team.
- Floating to different units within the Colby Campus, and potentially the Pacific Campus.
- Patient assessments.
- Treatment planning.
- Medication administration (if applicable based on unit assignment).
- Case management duties.

Required Skills & Certifications:

- Active Registered Nurse (RN) license in the state of employment.
- Minimum one year of experience in a hospital setting.
- Ability to work effectively in a fast-paced environment.
- Excellent communication and interpersonal skills.
- Interdisciplinary team collaboration skills.

Preferred Skills & Certifications:

- Two or more years of hospital experience.
- Experience in multiple units (ED, ICU, etc.).
- Bachelor's degree in Nursing or related field.

Special Considerations:

- Floating between units is REQUIRED within the Colby campus and may include the Pacific campus (1.7 miles away).
- Weekends are typically required every other weekend (EOW).
- RTO restrictions must be provided at the time of submission.
- Business casual attire.
- EPIC charting system used.
- Parking is provided by the facility.
