Location: Seattle, WA 98108

Duration: 13 weeks Contract (Strong Possibility of Extension)

Shifts: 08:00 - 16:30

Local Pay Range: $43 to $47/hr. on W2

Travel Pay Range: $46 to $51/hr. (Blended)

Job Description:

We are seeking an experienced Case Manager with a Master’s in Social Work and a Washington state social work license (WA LICSW, WA LSWAIC, or WA LASW) to join our outpatient community clinic team. In this role, you will conduct initial and 180-day assessments for participants, coordinate hospital and SNF discharges, and collaborate with an Interdisciplinary Team (IDT) to ensure safe, effective, and person-centered care.

This is a community-based case management position with moderate travel for participant home visits. Travel occurs during regular work hours, is considered paid time, and mileage is reimbursed when traveling between work locations (not for home-to-work commute). You will have autonomy in scheduling your home visits.

Key Responsibilities

  • Complete initial and 180-day participant assessments (clinic and in-home)
  • Coordinate hospital discharges by working closely with discharge planners
  • Communicate IDT recommendations for post-acute care
  • Arrange and coordinate SNF discharges for participants in rehab or skilled care
  • Develop, implement, and update individualized care plans
  • Facilitate care coordination between participants, caregivers, and community resources
  • Ensure documentation in EPIC meets regulatory and organizational standards
  • Conduct moderate in-field work (less than typical home health or hospice social work)

Minimum Requirements

  • Education & Licensure: MSW + one of the following: WA LICSW, WA LSWAIC, OR WA LASW (required upon submittal)
  • Certifications: BLS (Basic Life Support)
  • Minimum 1 year of experience in case management, discharge planning, or related role
  • Experience in outpatient, LTC, rehab, or acute care settings
  • Proficiency with EPIC
  • Knowledge of care coordination, community-based service programs, and adult protective services
  • Strong skills in biopsychosocial assessments, discharge planning, and crisis intervention
